All payments hereunder shall be made to DCA Productions Plus, Inc. and ARTIST

A non-refundable first payment of $ 500.00 shall be payable to DCA Productions Plus, Inc. (Fed ID# 13-0024434) and remitted to DCA Productions Plus, Inc., 57 Olde Noah Hunt Road, Millstone Township, NJ 08510. Please return the contract and payment within 60 days of receiving the contract.

The balance of $ 2,000.00 shall be paid not later than the day of and upon completion of the show as follows: Avner Eisenberg, SS # - 259-78-0184 for $1,125.00, Alfred J. Getler - 138-40-0290 for $ 875.00. All w-9s are attached.
